As a courtesy to you, our office will file a claim with your dental insurance company. However, we are in not in-network with any insurance company. This may result in benefits being paid directly to you. Please contact your insurance company for coverage and benefits. Payment for services is expected at the time of service. If you have dental insurance, we will collect your copay at the time of service. If you are unable to make this payment, please contact our office prior to your appointment to make arrangements.
